Utilization review procedure records, 1977-[ongoing].

The records are divided into two subseries. The first subseries consists of the Utilization Review Log and this is considered a permanent record of all reviews. The second subseries consists of Medicare, Medicaid, Blue Cross/Blue Shield certifications, utilization review sheets, weekly review schedules, denial letters, third party payee notification of coverage, quality record reviews, and related correspondence.
